The Submission Form
- Visit the Salesforce submission form at https://usd.tfaforms.net/218665
- Log in with your USDOne credentials.
NOTE: You'll notice the first three fields are pre-populated with your personal information.
Finish Your Identity Information
NOTE: Some fields will be pre-populated. All fields in this section of the form are required.
- Name and USD Email: not editable
- Title: Update your title if desired
- Degree/Credentials: Enter your degree and/or credentials
- School: Select your school from the list.
Choose Which Newsnotes Issue to Include the Varia In
- Newsnotes Issue: Select the
- Newsnotes Issue (month) you would like your submitted varia to appear in.
- Year: Select the year for the
- Newsnotes issue. You can choose from the current year or two subsequent years.
Add Varia Information
- Is this a publication/presentation/conference/media appearance? Please select the most appropriate option.
- Title:
- Title of item, if one exists
- Link (URL to specific item):
- Please include full URL, including https://, if this field is utilized.
- Description (1-2 sentences at most)
- Add another submission: Click "add another submission in the bottom right of this section to add multiple items to this
- Newsnotes issue.
After clicking "Add another submission"...
Submit the Form
- Submit: Click "Submit". You will receive a confirmation on the screen, and the Provost's Office will review and approve your submissions for inclusion in the desired newsnotes Newsnotes issue.
